Cape Cod Healthcare Seeks Community Benefits Grant Applicants

) Cape Cod Healthcare (CCHC) is pleased to release its 2022 Annual Strategic Grant Request for Proposals (RFP). The 2022 RFP is part of an annual competitive grant process focused on addressing the unmet health needs of the residents of Barnstable County.

The CCHC Community Benefits grants program is open to all non-profit community organizations operating in Barnstable County which have programs aligned with CCHC Community Benefits priorities.

The Annual Strategic Grant RFP focuses on the health priorities outlined in the 2020-2022 Cape Cod Hospital and Falmouth Hospital Community Health Needs Assessment Report and Strategic Implementation Plan.

This year’s applicants are invited to submit proposals for programs or projects that:
• Improve the physical and/or mental health of Cape Codders
• Address unmet public health needs
• Promote health equity

Projects that also address one or more of the following Social Determinates of Health will be given extra consideration: Transportation, Housing, Workforce Development.

Cape Cod Healthcare Community Benefits will award up to $300,000 total for programs executed January 1, 2022 through September 30, 2022, through a competitive grant-making process as outlined in this RFP packet. The maximum individual one-year award will be $30,000.

The deadline for submission of strategic grant proposals is September 15, 2021. Applicants will be notified of award in December 2021 with a funding period from January 1-September 30, 2022 and a summary report of outcomes due by November 1, 2022.

The CCHC Community Benefits program promotes the Cape Cod Healthcare mission to coordinate and deliver the highest quality, accessible health services to Cape Cod residents and visitors.
